1) The Legal Pain Registration Form 

2) The Legal Pain Wrestling Waiver 

3) The US Wrestling Medical Insurance And Waiver Form.

Membership Pricing And Payments 

  $100 a month.
  $150.00 a month. (includes 1 private a month and current singlet
1 day $20 drop in fee.
10% off for  parents that in active duty and veterans with proof.
All payments must made in person.

*30 day cancellation* Recurrent payments/ACH automatic payments w/ signature.
